Our referral forms can be downloaded below, and referrals are received via fax at 604-638-2916. We accept referrals from physicians or nurse practitioners, or for non-MSP insured patients we offer private consultations. Please complete our referral form or include a short letter detailing the nature of the patient’s concern. Once we receive your referral, our system will triage based on urgency and provide you a letter confirming your patient’s anticipated appointment date and time.

Note that all referrals sent to our clinic are pooled into central intake and will be booked with the next available physician.
